Kişisel Verilerin Korunması ve İşlenmesi Aydınlatma Metni
Bu metin bilgilendirme amaçlıdır.
1. Veri Sorumlusu
6698 sayılı Kişisel Verilerin Korunması Kanunu (“KVKK”) kapsamında kişisel verileriniz; veri sorumlusu sıfatıyla AHD Özel Sağlık Hizmetleri Güz. Merk. Med. Tur. İnş. Lok. San. ve Tic. Ltd. Şti. (“Özel AHD Polikliniği”) tarafından aşağıda açıklanan kapsamda işlenmektedir.
2. İşlenen Kişisel Veriler
İnternet sitemiz bilgilendirme amaçlıdır. Bizimle iletişime geçmeniz hâlinde ad-soyad, iletişim bilgileri (telefon, e-posta) ve tarafınızca iletilen bilgiler işlenebilir. Site üzerinde çevrim içi form veya ödeme alınmamaktadır.

5. Aktarım
Kişisel verileriniz, yalnızca yasal yükümlülükler ve hizmetin gerektirdiği ölçüde yetkili kamu kurum ve kuruluşları ile sözleşmeli hizmet sağlayıcılara KVKK’ya uygun olarak aktarılabilir.
6. Haklarınız (KVKK m.11)
Kişisel verilerinizin işlenip işlenmediğini öğrenme, işlenmişse bilgi talep etme, işlenme amacını öğrenme, eksik/yanlış işlenmişse düzeltilmesini, şartları oluştuğunda silinmesini/yok edilmesini isteme ve işlemeye itiraz etme haklarına sahipsiniz.
